Directions to Submit MMR and Meningitis Immunization Records
MMR (Measles, Mumps, Rubella)
If you are taking 6 or more credits in-person or hybrid and you were born after December 31, 1956, you are required to submit proof of vaccination for MMR. The first dose should be administered on or after your first birthday and the second dose must be administered at least 28 days after the first dose.
MEN (Meningitis)
Regardless of age, New York State Public Health Law 2167 requires all students taking 6 or more credits in in-person or hybrid courses to provide information about Meningococcal Meningitis vaccination. You must enter the date of your vaccine (called Menactra or MCV4) taken within the last 5 years or waive the vaccine.
Students who are required to and do not submit proof of immunization to the Student Health Center are subject to administrative withdrawal from in-person and hybrid courses.
How to Submit
If you have access to your Lehman360 account, you should click here to submit your proof of MMR and MEN immunization.
If you do not have access to your Lehman360 account, you should click here to submit your proof of MMR and MEN immunization. You will need to create an account to complete and submit the form.
Acceptable Proof of Immunization
All proof of immunization should have student’s name and date of birth on it. We cannot accept any proof of immunization that does not have the student’s name and date of birth displayed on it.
Acceptable proof of immunization includes the following:
- Vaccination card from childhood
- Immunization records from your doctor’s office (signed and stamped by doctor or clinic)
- High school or prior college’s immunization records
- Citywide or Statewide Immunization Registry print-out
- Blood Test, including original lab report proving immunity for MMR (Titer)

Recent CUNY Graduates, CUNY College to Lehman College Transfer and/or Readmitted Students Only
Intra-CUNY Transfer Students Only
If you are transferring to Lehman College from another CUNY institution, your immunization records should already be in CUNY First and, therefore, you will NOT need to resubmit your records and there should NOT be a negative service indicator on your record.
I need a copy of my MMR and Meningitis Immunization Records
You may obtain a copy of your immunization records remotely through the Student Health Portal.
Just log on to Lehman 360 using the firstname.lastname from your Lehman email and your password and click on the first app under Lehman Apps - Student Health Portal.
What do the NYS Public Health Laws require?
Measles, Mumps, and Rubella Requirements
Public Health Law 2165 requires that students born after 12/31/1956 provide proof of the following immunizations in order to register for classes.
- Measles – 2 immunizations given after 1968, after your first birthday and at least 28 days apart.
- Mumps – 1 immunization
- Rubella – 1 immunization
Acceptable Proof of Immunity
- Vaccination card from childhood
- Immunization records from your doctor’s office (signed and stamped by doctor or clinic)
- High school or prior college’s immunization records
- Citwide or Statewide Immunization Registry print-out.
- Blood test, including original lab report, proving immunity to measles, mumps, and rubella (MMR titer)
Records Requirements
All records must:
- clearly indicate the type of vaccine, dates of vaccine, name, and address of the doctor or clinic;
- be stamped and signed by the doctor or clinic; and
- be easily readable.
MMR vaccination is available at the Student Health Center free of charge. For students seeking MMR vaccine who do NOT have health insurance, you may receive free vaccine at the following Dept. of Health Immunization Clinic:

Meningitis Requirements
Public Health Law 2167 requires all college to provide information on Meningitis and the Meningitis vaccine. Regardless of your age, you will need to complete and sign the Medical Requirements form Part 1. If you have been vaccinated within the last 5 years, please upload your documentation. If you have not been vaccinated, you may decline the vaccine by checking the second box in Part 1 of the Medical Requirements form, sign and date.
The Health Center DOES NOT offer the Meningitis vaccine.
